The conversion factor can range from 0.3 to 0.8 units of insulin per kilogram. Also calculates daily insulin glargine, ie Lantus, dose based on total NPH dose. To convert, the total daily NPH is multiplied by 0.8 to get the daily night time Lantus dose.

To convert NPH to Lantus, enter the total NPH and hit convert.

Seek the advice of a licensed physician. Patients should not adjust their own insulin regimen. Hypoglycemia, also known as low blood sugar, can be fatal.
